Hi, I have a 98 Frontier 2WD with manual transmission and I'm wondering if
anyone else has had transmission problems with this year model. When my
truck had 45,000 miles about 4 years ago the reverse gear went out, so I
had the transmission replaced and now (60,000 miles later) the reverse gear
has started making a horrible noise and the fifth gear has gone out
completely- it acts as though its in neutral when in 5th gear- making
highway speeds impossible. Is this just a design defect with these
transmissions or could it be the way I drive (perhaps too fast at 80 mph on
the highway)?

This is honestly the first I'VE heard of there being problems with them. Of course I've been out of the loop. 5th and reverse are on the same slider, so if having problems with just those gears makes me suspect the slider for them is malfunctioning. Not really worth replacing the entire transmission, unless the shop is going to charge the same amount for a rebuild, as it would cost for a new one.

Sounds like someone is still rolling when going into reverse!!!!! Possible the mainshaft nut has also backed off. It's a relative easy fix, but with high labor rates and parts prices, a used trans may be the way to go.

With a used trans I would worry that it won't last long enough to make
the savings over a new one worthwhile. The labor cost is so high with
this type of job I'd rather get a new one and know what I'm getting.
On the other hand, the truck has 100,000 miles and I'm not sure how
much longer I should expect the engine to keep going. I know it has a
timing chain (as opposed to a belt) and I've been told that chains aren't
replaced unless they break (which of course would be a major problem),
and this worries me as well because of the high mileage and the last thing
I need is to put money into a trans only to have engine problems in the
near future. I've also been told that Nissans are hard to kill so I'm really
not sure if I should fix it or get rid of it...
Also, I'm always careful about being at a complete stop before changing
into reverse, but this all started when I had my clutch replaced and the
mechanic noticed a fluid leak out of the transmission. It was only about
half a quart low, and he filled it and the next day the reverse and 5th
started acting up. It's kind of strange the trans died right after getting a
new clutch and fluid refill, but I don't see how this could have affected it-
am I missing something here? I know it's not the mechanic's fault
because he has 25 years experience and I know him personally and trust
him but it's an odd coincidence.

Well a 98 with timing chain, so that would a KA24DE motor... capable of making well over 200,000 miles.
Sometimes you just run out of luck on certain parts. But with fluid leaking out of the transmission, did he just fill it up, or replace a seal at all?

It's a 3.5 hour job to remove and re-install the trans as per Motor All Data. A competent technician can easily do it in well under 2 hours, but that is beside the point. 7.5 hours to remove the trans, over-haul it , re-install. It does sound like a coincidence that it started acting up after the clutch replacement????? But it also has been run low on lube and at excessive high way speeds!!!!!! A tear down will give you the answer, it sounds like the mainshaft nut has backed off as mentioned earlier. Another very rare possibility is, when the trans was re-installed, the dowels were miss aligned, causing poor alignment to trans and engine. This was common on the v-6 4x4 trans due to the weight and difficulty of installation.(I have never had a problem and have had a lot of them out).
