My 2003 Nissan Frontier is squeeling as soon as I start it, when it is cold. The squeel only exists when the clutch is not engaged. As soon as, I engaged the clutch the squeel goes away. I can be driving and it seems worse in first and second gear but as soon as I push the clutch in it stops sqeeling. After driving for awhile and the truck is warmed up it is quiet. I was told that it may be the lack of grease under the boot fork and pivot ball?  Any suggestions?

You contradict yourself with that statement. When you start the vehicle with the clutch pedal depressed- no noise?? When you release the clutch-noise appears?? If it was the pilot bushing, it would squeal as you release the clutch pedal-usually only does it one time. If it squeals with clutch pedal released-most likely release brg.

Throw-out bearing(release bearing)
When you start the truck with the clutch depressed, does the noise appear just as you release the clutch or does the noise continue with the clutch released until you depress it again. Still not clear on how long it squeals and when.
