A while back I got some new tires on my 04. The shop did
a 4wheel alignment and balanced the tires. Ever since,
I've had an annoying shaking of the steering wheel under
acceleration and deceleration at different speeds.
I had the shop re balance my tires and rotate them. It
got better but the problem is still there.
While accelerating, between 35-45 mph the steering wheel
shakes. While decelerating, it shakes a little less but
still in the same speed window.
I'm pretty stumped as to what's going on.

It could be possible that there is some dirt between the rim and the contact area on the hub so that one or more of the wheels is not quite true. This can happen when removing the tires and rims to balance if a piece of dirt or debis loosens and moves to act like a shim. Have the tires and rims taken off and the backs cleaned and checked as well as the corresponding mounting surface on the vehicle.
Also make sure that none of the studs that hold the lug nuts is stripped. Again, a loose lug nut (or two or three) could allow the wheel to wobble.
Did you or anyone else run a rim into a curb hard enough to bend it? That can be another cause.

Howdy Partner...
It is possible to balance a tire that is out of round. Jack it up and check them over. Also tires that require over 2.5 oz of weight can have problems like yours.

The shop said the tires were out
of balance, and since they were
the last ones to touch
them...they are fixing it.
They did find that the bushing
that my drive shaft passes
through just after the
transmission is badly worn and
needs replacing. I asked if that
could cause the shaking, they
said possibly.
Any ideas? Also, is that bushing
something I can take care of
myself? Any special tools
required or is that thing
pressed on?

That "bushing" is the center carrier bearing that needs an
air hammer to remove. If it is not re-installed in the
correct "plane" with the front u-joint, you will get a real
bad vibration. Have you tried switching tires front to rear
to see if the problem goes away?

The shop just finished with my truck. I had them re balance
all the tires and clean the mounting surfaces when they did
my brake job. The shaking seems to be gone for now.
Thanks for the replies everyone! Now, on to center carrier
bearing. FUN!

Yeah, the guy at the shop said he used a lot of weight to
balance the tires. The tires are reasonably new and are
supposed to be good tires.
My Jeep had tires that were out of round. I almost
crapped my pants when that thing started vibrating one
day. It was almost like my front tires were dribbling
like basketballs. Once I bought new tires, no more
problems.
Anywho, the rims on my Frontier are powder coated, it
makes me wonder if maybe there are some variances in the
thickness of the coat that make the wheel hard to
balance. Maybe, maybe not.

In Jeeps, that is called "death wobble" and can be quite severe and serious. You can read about it on Jeep forums. Might be worth your while to look it up to see if it gives you any ideas for the future.

Road force is another issue when balancing tires, not many
shops have a machine that will measure road force-you can
actually match the tire(hard or high spot) to the wheel low
spot to minimize road force readings.
