I have a 2001 Nissan Frontier long bed club cab (4-door).
The lug nuts come loose and the wheels have almost fallen off twice.
Is there something wrong with the truck or is somebody tampering with the
lug nuts?
I drive a lot for work, and park in public areas and leave the truck for hours
at a time, and I go to the same locations on a regular basis, so it's possible.

Has anybody else had a similar problem that could indicate the problem is
mechanical? I have a lot of mileage on the truck, but have been OK with
maintenance.

Not that I am aware of, is it all 4 wheels, or is it the same specific wheel?

You might have to check the lug nuts before you go anywhere, it might be someone tampering that is causing this.

Are these factory or aftermarket wheels?  Different rims sometimes use different style lugnuts.  Some have a shoulder that fits into the rim (usually on a mag style wheel) and some have a tapered edge (most common with steel wheels).  Just make sure you have the correct lugnut for the rim style.  A friend of mine's son recently lost a wheel while traveling down the road and totalled his car for this very reason.  Good luck.

Wow, that is dangerous! If you check your lug nuts each time before you drive off, that might be a good idea. I would test by going somewhere that is outside your normal routine. I can't imagine somebody following you everywhere and tampering with your wheels. Also, try those lug nuts that requires a key. God speed.
