The bed cover will help out, especially since you drive 60-65. You could probably expect around a 3-4 mpg increase. The added weight of the 4wd kind of kills which is why you see 18 right now. Plus the cover will give you some security to store stuff.
Tire inflation is very important, just go by what Nissan recommends for your truck. There should be sticker in the driver's door jam. Make sure to check it cold, ie before you drive the truck around.
As for the exhaust, at the very least a smaller muffler to help free up some flow. You don't have to get a overly loud exhaust. Just find something you like the sound of. Youtube is a great way for this. The idea on this is the smaller the muffler, the louder the exhaust. So the larger the muffler, the quieter the exhaust. The stock muffler is very restrictive though, so just about any aftermarket will give you a very small gain. HOWEVER, one big concern is if your state does vehicle inspections (you only list US). I know people use aftermarket exhausts in strict states like NY and Cali, but I know some areas of those states are stricter than others. I'd hate to see you fork out a little bit of money for a newer muffler only to have the state fail it (if it applies to you)
Proper tire inflation and a bed cover are your best bets to increasing your mileage. The other items will just add on a little bit more but won't show the gains as much as those.
